No. It is a simple approval, dated and in the person's name, with no value as a qualified electronic signature, and the sign-off card reminds the client of this when they answer. If your field requires a qualified signature, use another tool.
You name them both. Each answers for themselves, and the sign-off is complete only when both have accepted.
They must give a reason, and the sign-off shows as “declined”. You see who answered and why, then submit a new sign-off if needed.
